Food Truck Request

Bringing a Food Truck to Campus:

Note: Campus does not allow food trucks to sell to the public on campus. If you want a food truck for your event, you or your department will have to pay for the truck to cater the event. Only under very certain circumstances will Dining approve food trucks to sell to the public on campus.

In order to have the food truck(s) you are bringing to campus approved, please complete the form below. Before you begin, please have the following documentation ready for upload for each truck at your event.

  • A copy of this agreement completed and signed by the food truck vendor
  • A copy of each food truck’s COI with NC State listed as additionally insured
  • A copy of their NC State health permit

If you have trouble with this form, or uploading files, please email Paul Matney at

    Who fills out the agreement?

Each food truck vendor you are bringing to your event needs to fill out the agreement

Why do I need to submit this paperwork?
To ensure that the food truck(s) is in compliance with NC State’s health and safety standards

When does the paperwork need to be submitted?
At least a week before the event for review

What happens if I can’t get paperwork from the food truck?
Any food truck who fails to submit the proper paperwork will not be allowed to serve food/beverages on campus

The food truck said they’ve already submitted this information before, do they really need to submit it again?
Yes, it is on both you and the food truck to submit paperwork for each truck and for each event. These documents should not be difficult to complete/submit.
